How To Request Shot Records from Tots & Tykes 🖍

Here is our guideline for requesting shot records ...

*Yes, we can fax a shot record. But first, we need a request from either the School or Daycare sent via fax to our office

*The request from the facility should include a Fax Cover Sheet or Letter Head, as long as it has their logo, phone number, fax number and address

*They need to write, “Fax over a shot record for ‘Patient Name & Date of Birth’”

*They can send it to our corporate office fax number

*We will fax the entire shot record back to the school, Head Start, or daycare facility

*We will not fax the shot record to a parent’s personal fax or place of employment

*A parent can also make an appointment to pick up the shot record
