Help! I need a Sports Physical for my child! We hear you and Tots & Tykes can help  🥎 Private insurance and Medicaid do not typically cover this expense, so this is self-pay and costs $35.00. Patients must be current with their Annual Well Checks -- don’t forget to bring the school form with you, so we can fill it out. We take care of kids up to 13 years of age at our Fort Worth and Arlington locations.
